Southside Support Documents
An mail from DocuSign, Inc. will arrive in your inbox within 10 business days. This e-mail will contain a link to the online support documentation that has to be completed and submitted along with an official transcript from the last school attended before the application process is considered complete and the application ready for evaluation. There are five support documents that will be available in the DocuSign, Inc. e-mail:
- Applicant Instruction Page
- Promissory Note
- Agreement To Terms
- Information Release Form
- Student Authorization Form.
In addition, the applicant must submit an official transcript from the last high school/college/university he/she attended.
Please monitor your e-mail to determine when an e-mail is sent from DocuSign, Inc. This e-mail will contain information as to how the support documents for your application are accessed and submitted. If you do not receive this e-mail within 7 to 10 working days following today's date, please contact Paul Farrar, Program Coordinator, at (434) 572-5484 or via e-mail at pfarrar@swcenter.edu.
